Explanation

Family practice repeat consultation: same consultant, same problem, after another physician cared for the patient in between. Needs a new written request.

What it covers

A repeat consultation is an additional consultation by the same consultant for the same presenting problem, after another physician has cared for the patient in the interval since the first consultation (GP19).

It has the same requirements as a consultation, including a new written request from the referring physician, nurse practitioner or dental surgeon and a written report back (GP16, GP19). A006 is the Family Practice & Practice in General code (A3).

Billing rules

  • Needs a new written, signed request naming the consultant or specialty, the referrer and billing number, and the patient and health number, kept in the chart. Otherwise it is paid as a lesser assessment (GP16, GP19).
  • Repeat consultations don't count toward the consultation limits (GP18).
  • If the repeat consultation requirements aren't met, it is paid as a general or specific assessment (GP17, GP18).
  • Not on the Appendix J list, so it can't be billed by video or telephone (A68, AJ1, AJ2).
  • Detention (K001) can only start after 60 minutes of a repeat consultation (GP30).
  • The E432 speculum pelvic exam add-on is listed only for A001 and A007 in Family Practice, not for A006 (A4).
  • In hospital the code is C006 (A12).

Often billed with

  • E430 (with G365) or E431 cervical cancer screening collection outside a hospital or ICHSC: the collection is otherwise included in a repeat consultation when a pelvic exam is a normal part of it (A4).
  • E079 smoking cessation initial discussion, when the physician is the primary care physician most responsible for the patient's ongoing care; A006 is on its eligible list, one per patient per 12 months (A56).

Common rejections

  • AC4: The referring provider number was missing, mistyped, your own number, or from a provider type that can't refer. A repeat consultation needs a new written request, so enter the six-digit number of the provider who made it (GP19).
