An internist or gastroenterologist bills A120 for an assessment on the day of a colonoscopy when the decision-to-scope visit was in the past 12 months.

Watch for: Only payable to Internal Medicine (13) or Gastroenterology (41), and not when the major pre-operative visit is the same day.

Assessment by a gastroenterologist or internist on the day of a colonoscopy, when the decision-to-scope visit already happened in the past 12 months.

What it covers

A120 is an assessment on the same day as a colonoscopy (A104).

It is the only assessment payable that day when a major pre-operative visit (the consultation or assessment where the decision to do the procedure was made) was rendered by any physician in the 12 months before the colonoscopy (A104, GP4).

Billing rules

• Only payable to Internal Medicine (13) or Gastroenterology (41) (A104).
• Only when a major pre-operative visit was rendered by any physician in the 12 months before the colonoscopy; it is then the only assessment payable that day (A104).
• Not payable if a major pre-operative visit is payable on the same day as the colonoscopy (A104).
• A pre-colonoscopy consultation is only payable if the chart shows it was medically necessary (GP18).

Common rejections

• A4D: A120 is only payable to Internal Medicine (13) or Gastroenterology (41); a claim under another specialty is rejected (A104).
