Explanation

Updated October 26, 2026

Add-on to S626 vasectomy when the vasectomy is done outside hospital, for example in the office.

What it covers

E545 is an add-on printed under S626, vasectomy (unilateral or bilateral, by any technique), payable when the vasectomy is performed outside hospital (U6).

It is specific to vasectomy, not a general office-procedure fee. Other procedures have their own outside-hospital add-on where the Schedule prints one, most often E542 (for example M15).

Billing rules

  • Payable only with S626, and only when the vasectomy is done outside hospital (U6).
  • S626 covers a unilateral or bilateral vasectomy by any technique, so a bilateral vasectomy is one S626 with one E545 (U6, SP4).
  • S626 is a surgical (S) code, not an IOP: the major pre-operative visit where the decision to operate is made is billed separately, whatever the time between that visit and the surgery (SP2, GP4).
  • A local anaesthetic or nerve block you give for your own vasectomy is not paid separately (SP5).
  • Vasectomy reversal (S623, S625) is insured only to treat significant symptoms and needs prior approval; re-establishing fertility doesn't count (U6).
  • FHO: neither S626 nor E545 is listed in Schedule 2 (core services), so they are not part of the base rate basket (Agreement pp.75–82).

Often billed with

  • S626 vasectomy, the only code E545 is listed under (U6).
  • The consultation or assessment at the visit where the decision to do the vasectomy is made, as the major pre-operative visit (SP2, GP4).
