Office quantitative HCG test for physicians whose practice needs point-of-care reproductive testing. Not payable as a pregnancy test (G005).
What it covers
G021 is a quantitative HCG (human chorionic gonadotrophins) test done in the physician's office (J65). It is in the Reproductive medicine group of Laboratory Medicine in Physician's Office (J65). It is not the code for a pregnancy test, which is G005 (J65).
Billing rules
- Reproductive medicine tests are payable only to physicians whose practice needs point-of-care testing (J65).
- Insured only when you, or a physician substituting for you, do the test in your own office for your own patient (J65).
- The chart must record the result, your interpretation and the treatment decision based on it, or the test isn't payable (J65).
- Not insured when done to support in-vitro fertilization or artificial insemination (J65).
- Not payable for pregnancy tests. A pregnancy test is G005, which is insured only when an immediate determination of pregnancy is needed to prevent imminent physical harm to the patient (J65, J67).
- Not marked (+), so the G700 sole-visit premium does not apply (J3, J65).
- Not on the GP58 list of exceptions, so G021 is not eligible for payment when the same physician renders any counselling service (K013, K033, K040, K041) to the patient that day (GP58).
Often billed with
- A prenatal assessment such as P003 or P005 the same day; office lab tests are payable in addition to a consultation or assessment (J3).
