Explanation

Updated October 26, 2026

Ultrasound-guided diagnostic lumbar facet medial branch block, first site. Needs your own fluoroscopic facet block at the same site within 12 months.

What it covers

G914 is a percutaneous diagnostic lumbar facet medial branch block done under ultrasound guidance, for the first site; each additional site is G915. The fluoroscopically guided versions are G910 (cervical), G911 (thoracic) and G912 (lumbar/sacral) (J75).

It is an interventional pain injection, payable only when done solely to diagnose the source of pain or to develop a treatment plan. The fee includes injection of contrast, medication or other solution, and 'site' means the anatomic area in the code's description (J74).

Billing rules

  • Payable only when you rendered a fluoroscopically guided facet injection for the same site(s) in the previous 12 months (J75).
  • Maximum 6 G914 per patient per 12 months; more needs written prior authorization from the Ministry, showing the frequency is generally accepted as necessary (J75).
  • G915 (each additional site) is limited to 7 services per patient per day (J75).
  • No separate fee for obtaining or interpreting the guidance images (J74).
  • Chart the procedure with images of final needle placement, plus the purpose of the block and the patient's response, positive or negative. Ultrasound images must clearly show the injection site and needle placement at the junction of the transverse process and superior articular process (J74, J75).
  • Other injection codes, such as joint injections G370/G371, can't be added for the same injection, and another procedure from this section the same day is payable only for a separate condition (J74).

Often billed with

  • G915 for each additional site in the same session (J75).
