Rotation flap, 2.1–5 cm, body
Bill R075 for a rotation, transposition or Z-plasty flap closing a defect of 2.1 to 5 cm outside the face, neck and scalp.
Watch for: Claim the flap instead of the excision code when it closes a lesion excision; an S-plasty counts as simple excision.

Rotation, transposition or Z-plasty flap closing a defect of 2.1 to 5 cm average diameter on the other areas.
What it covers
R075 is a skin flap by rotation, transposition or Z-plasty for a defect of 2.1 to 5 cm average diameter on the other areas. Undermining is included (M17).
Payment depends on the size and location of the area and the type of flap or graft; other procedures besides the skin flap or graft (for example tendon grafts) are payable in addition (M17).
Billing rules
- When a benign, pre-malignant or malignant skin lesion is excised and closed by rotation, transposition, Z-plasty, flap or graft, claim this repair code instead of the excision code (M17).
- Closure by S-plasty is a simple excision, not a reconstructive flap (M17).
- More than one procedure under the same anaesthetic: full fee for the major procedure, 85% for the others (SP2 §3).
- Surgery only to change appearance is insured only in the cases set out in Appendix D (SP5, AD1).
- Marked #: the fee does not include premises, equipment, supplies or staff; in hospital these are funded by the hospital budget, and patients can't be charged for them (GP13).
- Age premiums apply for patients under 16, from 10% to 30% by age (GP64).
Often billed with
- E540 (+25%), once per lesion, for excision in hospital for a tumour-free margin with frozen section, added to the first flap or graft (M17).
