Explanation

Full-thickness skin graft, intermediate, 1 cm to 5 cm average diameter.

What it covers

R093 is a full-thickness skin graft, intermediate, 1 cm to 5 cm average diameter (M19). Full-thickness grafts are R092 (under 1 cm), R093 (1 to 5 cm), R083 (over 5 cm) and R091 (eyelid, nose, lip or face) (M19).

Payment depends on the size and location of the grafted area and the type of graft; other procedures besides the graft are payable in addition (M17).

Billing rules

  • The chart must show that the code claimed fits the size of the graft (M19).
  • Skin grafts are not payable in addition to R117 (skin-sparing mastectomy with immediate reconstruction) (M19).
  • When a lesion excision is closed by a graft, claim the repair code instead of the excision code (M17).
  • More than one procedure under the same anaesthetic: full fee for the major procedure, 85% for the others (SP2 §3).
  • Marked #: the fee does not include premises, equipment, supplies or staff; in hospital these are funded by the hospital budget, and patients can't be charged for them (GP13).
  • Age premiums apply for patients under 16, from 10% to 30% by age (GP64).

Often billed with

  • E540 (+25%), once per lesion, for excision in hospital for a tumour-free margin with frozen section, added to the first flap or graft (M17).
