General re-assessment by an endocrinologist of a patient re-admitted to long-term care after at least 3 days in another institution.
What it covers
A Type 3 admission assessment is a general re-assessment of a patient re-admitted to the long-term care institution after a stay of at least 3 days in another institution (GP49).
A general re-assessment includes everything in a general assessment, except the history need not repeat details already obtained (GP22). W-prefix codes are for non-emergency care in chronic care and convalescent hospitals, nursing homes, homes for the aged and designated chronic or convalescent beds, but not designated palliative care beds (A103, GP49).
Billing rules
- Only for a re-admission after a minimum 3-day stay in another institution (GP49).
- Use Type 1 (W252) or Type 2 (W254) for a first admission (GP49).
- Not on the virtual care list, so not payable by video or phone (Appendix J).
