Explanation
Updated October 26, 2026
Special family practice consultation of at least 50 minutes for a patient in a long-term care institution.
What it covers
W911 is a consultation by a GP/FP who provides all the elements of a consultation and spends at least 50 minutes in direct contact with the patient, not counting other separately billable services (A1). It is subject to the same conditions as A911 (A19).
Time excludes non-patient-facing work such as reviewing charts, imaging or documentation (A1).
Billing rules
- Record start and stop times in the chart, or it is paid at a lesser fee (A1).
- No other consultation, assessment, visit or counselling service is payable the same day for the same patient by the same physician (A1).
- The written, signed request must name the consultant or specialty, the referring provider and billing number, and the patient and health number; in an LTC institution it can be on the common medical record. Otherwise it is paid as a lesser assessment (GP16).
- Counts toward the consultation limits, like any time-based consultation (GP18).
- Detention (K001) can start only after 90 minutes (GP30, GP31).
Common rejections
- ARF: 'Missing physician referring number': a consultation claim needs the referring physician's or nurse practitioner's billing number (GP16).
