Incision and drainage of an ischiorectal or pilonidal abscess or haematoma under local anaesthetic. Under general anaesthetic, bill Z107.

What it covers

Z106 is incision and drainage of an ischiorectal or pilonidal abscess or haematoma under local anaesthetic (M1). The same under general anaesthetic is Z107, and a perianal abscess is Z104 (local) or Z105 (general) (M1).

As a Z code it is an independent operative procedure, payable in addition to an assessment (GP4). Excision or marsupialization of the pilonidal cyst itself is R035 (M4).

Billing rules

  • Bill under local anaesthetic only; with general anaesthetic use Z107, with E515 when it is the sole procedure in an operating room outside the emergency department (M1).
  • Add E542 when performed outside hospital (M1).
  • If you did the major pre-operative visit in the previous 12 months, only a minor assessment (GP/FP) or partial assessment (specialist) is payable before the drainage that day; if the decision is made at that day's visit, no other assessment before the IOP (SP4).
  • For a non-elective procedure done on a special visit, bill the consultation or assessment, the special visit premium and the procedure (SP2).
  • FHO: not in basket (not listed in Schedule 2), but counts toward the Schedule 4 special payment for office procedures done in your office for enrolled patients (Agreement pp.75-82, p.67 §8.7, p.94).

Often billed with

  • A034 or A035 General Surgery partial assessment or consultation at the visit where the decision to drain is made (GP4, SP4).
  • U990 or U991 hospital out-patient special visit premiums for a non-elective drainage on a special visit that meets the Table II rules (SP2, GP65, GP71).
  • E542 when done in the office (M1).
