What it covers
Z757 is excision of one or more benign anal lesions, listed under Rectum (S28). Local excision for malignancy is S249, and fulguration of condylomata is Z549 (local) or Z758 (general anaesthetic) (S28).
As a Z code it is an independent operative procedure, payable in addition to an assessment (GP4).
Billing rules
- Add E542 when Z757 is performed outside hospital (S28).
- Surgical assistant: 6 basic units (B suffix); anaesthesia: 6 basic units (C suffix) (S28).
- If you did the major pre-operative visit in the previous 12 months, only a minor assessment (GP/FP) or partial assessment (specialist) is payable before the procedure that day; if the decision to operate is made that day, that visit is billable but no other assessment before the procedure (SP4).
- When the lesions treated are intra- or perianal condylomata acuminata, the fee includes anoscopy, so Z543 isn't added (S27).
Often billed with
- E542 when done in the office or another setting outside hospital (S28).
- A034 partial assessment or A035 consultation, subject to the IOP visit rules (SP4, A107).
