Error code

09 — Incorrect fee code. Resubmit with the correct FSC, or submit an RAI if it's already on the Remittance Advice

Wrong fee code

OHIP considers the fee code on the claim wrong for the service that was provided, so it wasn't paid as billed.

What to do: Find the right code in the Schedule of Benefits; resubmit if on the Error Report, or send an inquiry if on the remittance advice.

Code
09
Description
Incorrect fee code. Resubmit with the correct FSC, or submit an RAI if it's already on the Remittance Advice

Explanation

Updated October 26, 2026

The fee code used isn't correct for the service. Resubmit with the right code, or send an RAI if the claim is already on your Remittance Advice.

What it means

Code 09 means OHIP considers the fee schedule code (FSC) on the claim to be wrong for the service. The Ministry's instruction is to resubmit the claim with the appropriate code, or to submit a Remittance Advice Inquiry (RAI) if the claim is already posted on a Remittance Advice (RA).

The same message is also used on the Error Report. Which route you take depends on where the claim shows up: a rejected claim on the Error Report is corrected and resubmitted, a claim on the RA is handled through an RAI.

Common causes

  • A code for the wrong setting, for example an office (A-prefix) code used for an in-patient service that has a C-prefix code
  • A code that doesn't describe what was done, often one left in a template or favourites list
  • A typing error in the fee code
  • The wrong suffix (A, B or C) for the role the physician played

How to fix it

  1. Check the Schedule of Benefits for the code that matches the service, the setting and the physician's role.
  2. If the claim came back on the Error Report, correct the fee code and resubmit. Resubmit within three months of the service date if you can; if that window has passed but the claim was first submitted on time, use the stale-dated process described under VJ7.
  3. If the claim is on the RA, don't send a new claim. Send an RAI through eSubmit, or on form 0918-84, within 7 months of the RA date, naming the code that should have been paid.

Don't resubmit if

  • You haven't changed the fee code. The same claim will come back again.
  • The claim is on the RA. Use an RAI instead.
