Code
EQ9
Description
Lab Number not on File

Explanation

Updated October 26, 2026

EQ9 means the laboratory number on the claim isn't on the Ministry's file. Check the lab number and resubmit.

What it means

EQ9 means the laboratory number on the claim does not exist in the Ministry's records, so the claim was rejected.

Common causes

  • A typing error in the lab number
  • A number that is not a Ministry lab number was entered in the lab field

How to fix it

  1. Check the lab number against the requisition or lab report.
  2. Correct the number on the claim.
  3. Resubmit the corrected claim within three months of the service date. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

How APEX handles it
