Code
MC
Description
Maximum case conferences reached for 12 months

Explanation

Updated October 26, 2026

MC means the patient already has the maximum case conferences paid in 12 months, usually 4 per patient per physician.

What it means

MC means the maximum number of case conferences for this patient has been reached in a 12-month period.

Most case conference codes (K121, K124, K700, K701, K703, K704, K705, K707, K714, K715) are limited to 4 services per patient, per physician, per 12 months. K706 is limited to 8. Each also has a daily maximum of 8 units (Schedule A33 to A41).

Common causes

  • A fifth case conference for the patient in 12 months
  • Several case conference codes counted toward the limit

How to fix it

  1. Count the case conferences paid for the patient in the 12 months before the service date.
  2. Check the limit for the code you billed (Schedule A33 to A41).
  3. If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The 12-month limit was already reached.
  • The line is already on your RA. Question it with an RAI, not a new claim.
