Error code
Q8 — Lab not licensed to perform this test on date of service
Lab not licensed for test
The laboratory wasn't licensed to perform this test on the service date, so OHIP didn't pay it.
What to do: Check the test code and service date, and whether the laboratory's licence covered that test on that date.
- Code
- Q8
- Description
- Lab not licensed to perform this test on date of service
Explanation
Updated October 26, 2026
Q8 means the laboratory wasn't licensed to perform this test on the service date, so OHIP didn't pay it.
What it means
Code Q8 means OHIP refused payment because the laboratory that billed the test was not licensed to perform that test on the date of service. The Ministry gives no further detail.
Common causes
- The test is not on the laboratory's licence
- The service date falls outside the licence period
- A wrong service date or test code on the claim
How to fix it
- Check the test code and the service date on the claim.
- Check that the laboratory's licence covered that test on that date.
- If the date or code was wrong, or the licence did cover the test, send an RAI with the details. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The laboratory was not licensed for the test on the service date.
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
