Initial procedure not found

OHIP couldn't find the initial procedure that this follow-up or subsequent procedure depends on.

Code
S9
Description
Initial procedure not found

Explanation

Updated October 26, 2026

S9 means OHIP couldn't find the initial procedure that this follow-up or subsequent procedure depends on.

What it means

Code S9 means OHIP refused or adjusted this line because it couldn't find the initial procedure in its records. Subsequent or follow-up procedure fees are paid only after an initial procedure.

Common causes

  • The initial procedure hasn't been billed, or was rejected
  • The initial procedure was billed under a different health number or physician number
  • The subsequent code was used for what was really the first procedure

How to fix it

  1. Check whether the initial procedure was billed and paid for the same patient.
  2. If it was never billed, bill it (if it was rendered). If it was rejected, fix that claim first.
  3. If this was really the first procedure, the initial code was the right one.
  4. Once the initial procedure is on file, send an RAI for this line. Send it through eSubmit, or on form 0918-84, within 7 months of the RA date.

Don't resubmit if

  • The claim is already on your Remittance Advice. Use an RAI, not a new claim.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
