- Description
- Major pre-op visit already billed
Explanation
Updated October 26, 2026
SC means a major pre-operative visit was already claimed for this surgery, so this one wasn't paid.
What it means
Code SC means OHIP refused this visit because a major pre-operative visit has already been claimed.
The major pre-operative visit is the consultation or assessment where the decision to operate is made, whatever the time between that visit and the surgery (Definitions, GP4). It is billed separately from the surgical fee (SP2). Other routine pre-operative care is part of the surgical fee.
Common causes
- A second pre-operative visit billed as if it were the decision-to-operate visit
- Another physician already billed the major pre-operative visit for the same surgery
How to fix it
- Find the earlier major pre-operative visit for the same surgery.
- If it was paid, this visit is routine pre-operative care and is included in the surgical fee.
- If the visit was for a different problem or a different surgery, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- This was a further visit about the same surgery.
- The claim is already on your Remittance Advice. Use an RAI, not a new claim.
