- Description
- Paid as extra patient seen in the home
Explanation
Updated October 26, 2026
Only the first patient seen on a home visit gets the home premium or A900. This patient was paid as an extra patient seen in the same home.
What it means
The Ministry's wording for V2 is "Allowed as extra patient seen in the home". OHIP paid this claim as an additional patient at the same home visit, not as the first patient seen.
Special visit premiums for a patient's home (Table VI, such as B960–B964 travel and B990, B992, B993, B994, B996 first person seen) are only payable for the first patient seen, however many patients are seen during one visit to a home or to units in one multiple resident dwelling, such as an apartment block or retirement home (GP75, note 3). The complex house call assessment A900 is also only payable for the first person seen during a single visit to the same location (A6).
Common causes
- Home visit premiums were billed for more than one family member seen in the same home
- Several residents of the same apartment building or retirement home were seen on one trip and each claim had the premium
- A900 was billed for more than one patient at the same location
How to fix it
- Check which patients were seen on the same trip to the same home or building.
- Bill the home premiums and A900 only on the first patient seen. Bill the other patients with the appropriate assessment code only.
- If the patients were seen on separate visits, keep the visit times from the chart.
- If you believe the payment is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- The claim is already on the RA. Use an RAI, not a new claim, if you disagree with the payment.
- The patients were seen during one visit to the same home or building. The adjustment is correct.
