Invalid Hospital Number

Explanation

Updated October 26, 2026

V28 means the hospital (master) number on the claim isn't a valid four-digit facility number. Correct it and resubmit.

What it means

Every hospital and certain other health facilities have a four-digit master number assigned by the Ministry. It goes in the claim header so OHIP knows where the service was given. V28 means the number on the claim isn't a valid one in the Ministry's Master Numbering System, so the claim was rejected to your Error Report.

The Ministry's claims specification says a master number must be present for C, H and W prefix fee codes, and when the service location indicator is HDS, HED, HIP, HOP, HRP or RTF. For example, non-emergency hospital in-patient services need the master number and the admission date. Emergency department visits and long-term institutional care need the master number.

A missing number (rather than a wrong one) usually rejects with a different code, such as AH8 for an in-patient claim without its admission date or master number.

Common causes

  • A typing error in the four-digit number
  • The facility number (a different number in the facility listing) entered instead of the master number
  • An old or default hospital number saved in the billing software for that location
  • A number that wasn't valid on the service date, for example after a facility change

How to fix it

  1. Check the master number for the hospital or facility where you saw the patient, using the Ministry's Master Numbering System list or the hospital's billing office.
  2. Correct the number on the claim. For in-patient services, also check the admission date.
  3. Update the default master number in your billing software so other claims from that site don't reject the same way.
  4. Resubmit the corrected claim within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

Don't resubmit if

  • You haven't changed the master number. The claim will reject with V28 again.
  • The service wasn't in a hospital or facility. Check whether the claim needs a master number at all, and whether the fee code fits the setting.

How APEX handles it

APEX lists V28 claims in the Correction Centre. For some family practice claims it drafts a corrected claim: check the drafted codes against the chart before you approve it. If you've turned on auto-fix, drafted corrections are sent without waiting for you. Other V28 claims are listed under Outstanding for your input. See how APEX works · Pricing
