- Description
- Only 1 geriatric assessment premium per patient within 12 months
Explanation
Updated October 26, 2026
OHIP allows only one geriatric general assessment premium per patient per 12 months. The premium on this line was refused.
What it means
Code VG means a geriatric general assessment premium was not paid because one has already been paid for this patient in the previous 12 months.
The Ministry's description gives no more detail. The other services on the claim are assessed separately.
Common causes
- A second premium claimed for the same patient within 12 months, by you or another physician
How to fix it
- Check whether the premium was paid for the patient in the past 12 months.
- If it was, there is nothing to fix.
- If you believe the payment is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- You disagree with how a claim already on the RA was paid. Use an RAI, not a new claim.
