FHO+ calculator 2026
Estimate a year of FHO+ income from your roster, hours and visits. FHO+ took effect on April 1, 2026: capitation is still the base, with hourly pay for Q310A–Q313A, a 30% shadow billing premium on in-basket services and a 50% after-hours premium.
Everything is worked out in your browser. Nothing you enter is sent or saved.
How the estimate works
Capitation. Each enrolled patient earns the annual FHO+ base rate for their age and sex (Schedule 1 of the FHO agreement, April 2026). Patients living in long-term care earn a flat $1,223.22 a year instead. Patients past 2,400 are counted at half the rate.
Hourly pay. Q310A direct care, Q312A indirect care and Q313A clinical administration pay $80 an hour, in 15-minute units. The calculator adds the most indirect and administrative time the mix limits allow (Q312A + Q313A no more than 25% of hourly time, Q313A no more than 5%), caps each day at 14 hours and each month at 257.1 hours (a 30-day month).
Shadow billing. In-basket services to enrolled patients pay 30% of the fee. The 44 Schedule 2B procedures, such as Pap tests and immunizations, pay 50%.
After-hours premium. Evening, weekend and holiday visits with enrolled patients on the listed codes add 50% of the fee with Q012A.
Other fee-for-service. Out-of-basket services and services to non-enrolled patients are paid at the full fee. In-basket services to non-enrolled patients have a yearly group cap.
Relativity. FHO+ adds the 2026–27 FHO relativity rate of 4.8506% to capitation, shadow billing, after-hours premiums and fee-for-service claims.
Continuity of care. If fewer than 75% of your enrolled patients' in-basket visits are with you or your group in two linked quarters, 15% of a quarter's base rate is taken back. The calculator shows that amount when your continuity is below 75%.
Not included: complexity-based capitation (the Ministry hasn't published the amounts), Q311A phone care away from the clinic ($68 an hour), the Enhanced Group Management and Leadership Payment, annual special payments, the preventive care bonus, age premiums and the yearly cap on non-enrolled in-basket billing.
Questions
How much does FHO+ pay per hour?
Q310A direct care, Q312A indirect care and Q313A clinical administration pay $80 an hour ($20 per 15-minute unit). Q311A phone care when you're away from your usual clinic pays $68 an hour ($17 a unit).
How many hours can I bill under FHO+?
Up to 14 hours a day across Q310A–Q313A, and a monthly limit set by the billing month's length: 240 hours for 28 days, 248.6 for 29, 257.1 for 30 and 265.7 for 31. Q312A and Q313A together can't exceed 25% of your hourly time, and Q313A can't exceed 5% of your direct and indirect care.
What is the FHO+ shadow billing rate?
30% of the fee for in-basket services to enrolled patients, up from 19.41% before April 1, 2026. 44 procedures in Schedule 2B pay another 20%, for 50% in total.
Is FHO+ better than FHG or CCM?
It depends on your roster, hours and how much of your work is in the basket. This calculator estimates FHO+ only. APEX's Group Switch Analysis compares FHO+, FHG, CCM and Income Stabilization side by side from your own remittance advice.
What happened to the FHO access bonus?
The access bonus and outside use deductions ended on April 1, 2026. FHO+ measures continuity of care instead, with a 75% target.
