OHIP billing code for disability tax credit form

Updated October 26, 2026

There is no OHIP billing code for the Disability Tax Credit Certificate (CRA form T2201). Completing it is an uninsured service, so you bill the patient directly, not OHIP. The OMA suggests charging your hourly rate, with a suggested minimum of $150.

How to bill it

Don't send a claim to OHIP for the form. Forms and reports completed for a third party, and the exam done only to complete them, are uninsured services (Regulation 552, s.24; OMA uninsured services guide).

Bill the patient directly. Tell them the fee before you do the work; charging more than the OMA's published schedule of fees without telling the patient first is professional misconduct under Regulation 856/93 under the Medicine Act (OMA uninsured services guide, p.6).

If you also treat a medical problem at the same visit, that care is still insured and billed to OHIP as usual. Only the form work and any exam needed just for the form are uninsured.

Billing rules

  • Never bill the patient for an insured service. 'Not eligible for payment' on a claim still means insured.
  • Physicians may not charge patients or third parties for keeping medical records, for conferring with other professionals about the patient, or for obtaining consents (OMA uninsured services guide, p.6, citing Regulation 552 s.24).
  • OMA suggested fee for the T2201: the physician's hourly rate, suggested minimum $150 (OMA uninsured services guide, January 2024, p.24).
