OHIP billing code for Form 3

Updated October 26, 2026

The OHIP billing code for a Form 3 is K624A, certification of involuntary admission under the Mental Health Act, $164.50; later re-certifications are K629A ($48.75). Form 3 is completed by the attending physician at the psychiatric facility, who can't be the physician who signed the Form 1, so a family physician bills it only when acting as that attending physician.

Bill K624A for the first certificate. It includes the history, examination, notifying the patient, family and relevant authorities, and completing the form (A24, GP34).

Bill K629A for each later re-certification.

Don't also bill an assessment the same day: consultations or assessments claimed in addition to certification or re-certification the same day pay nil (A25).
