Periodic eye and vision exam for simple refractive error in patients 65 and over. One per 12 months across all physicians and optometrists.

What it covers

A periodic oculo-visual assessment is an exam of the eye and vision system done mainly to find out whether the patient has a simple refractive error (myopia, hypermetropia, presbyopia, anisometropia or astigmatism). A112 is the code for patients aged 65 and over (A10, GP28).

It includes all the components needed, ordinarily history of the presenting complaint, past medical history, visual acuity, ocular mobility, slit lamp exam of the anterior segment, ophthalmoscopy and tonometry, plus advice to the patient and a written refractive prescription if needed (GP28).

Billing rules

  • One per patient per 12 months, whether the earlier one was by you, another physician, or an optometrist's major oculo-visual examination. Services over the limit are not insured (GP28).
  • Any other insured service by the same physician (other than an ophthalmologist) to the same patient on the same day is not payable (GP28).
  • Other consultation or visit codes are not to be used in its place when the limit has been reached (GP28).
  • A re-assessment after a periodic oculo-visual assessment is claimed with a lesser assessment code and diagnostic code 367 (GP28).
  • FHO: A112A is an in-basket core service (Agreement Schedule 2, p.75).
