What it covers
E077 is the service of determining that a patient aged 20 to 64 inclusive has a medical condition that needs a major eye examination, and giving the patient a completed requisition (A10).
It is for conditions other than diabetes mellitus, glaucoma, cataract, retinal disease, amblyopia, visual field defects, corneal disease, strabismus, recurrent uveitis or optic pathway disease. Patients with those conditions are already eligible for a major eye exam (A115) without a requisition (A10, A11).
Billing rules
- Only for patients aged 20 to 64 inclusive (A10).
- One every four fiscal years by the same physician for the same patient, unless the patient goes for the exam to someone other than the one the original requisition was given to (A10).
- One per fiscal year per patient by any physician (A10).
- The requisition stops being valid after March 31 of the fifth year following the year it was completed (A11).
Often billed with
- E077 is listed as an add-on, claimed with the visit at which the need for a major eye exam is identified, such as A007 or A003 (A10).
