Follow-up vision rehabilitation visit by an ophthalmologist for a patient with low vision, after an initial A252. At least 3 of 8 listed components.
What it covers
A follow-up vision rehabilitation assessment for a patient with low visual acuity, a visual field defect or significant oculomotor dysfunction that is not amenable to further medical or surgical treatment (A157). Low visual acuity means best corrected acuity of 20/50 (6/15) or less in the better eye (A157).
At least 3 of the 8 components listed for the initial assessment must be done at the same visit. They include assessing residual visual function, eccentric preferred retinal loci, near functional acuity and reading skills, prescribing low vision devices, preparing a rehabilitation plan and supervised training (A157).
