Error code
AM1 — Service Limit Exceeded
Over the service limit
The service goes over a Schedule of Benefits limit for this patient, such as a maximum per day or per year.
What to do: Check the patient's history for the period; if over the limit it isn't payable, so correct only wrong dates or counts.

- Service Limit Exceeded
Explanation
Updated October 26, 2026
AM1 means the service is over its Schedule limit for this patient (per day or per period). Services over the limit aren't paid.
What it means
AM1 means the claim was rejected because it goes over a limit in the Schedule of Benefits for this service, such as a maximum per day, per year or per pregnancy.
When a maximum has no time period, it means per patient per day. When it names a period, services are counted per patient in date order, and those over the limit are not eligible for payment (GP7). Some limits count services by any physician; others count only your own (for example, one periodic health visit per patient per physician per 12 months, GP21).
Common causes
- The patient already had the maximum number of this service in the period
- More services on the claim than the daily maximum
- The same service billed twice by mistake
- A wrong service date that puts the service inside an earlier period
How to fix it
- Look up the limit for the code in the Schedule.
- Check the patient's billing history for the service in that period, including other physicians if the limit applies to all.
- If the claim is over the limit, it isn't payable.
- If the date or number of services was wrong, correct it.
- If you provided a different service that has its own code, bill that code instead.
- Resubmit the corrected claim within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- The service really is over the limit.
- You would only be swapping in another code to get around the limit. Bill only codes that describe what you did.
How APEX handles it
