Explanation

Updated October 26, 2026

Assessment by a respirologist for ongoing management of complex conditions such as chronic respiratory failure, bronchiectasis, cystic fibrosis or TB.

What it covers

A570 is an assessment for ongoing management of a respiratory condition complex enough to need continuing care by a respirologist (47): chronic respiratory failure (symptomatic, PaO2 < 60 mmHg and/or PaCO2 > 50 mmHg), bronchiectasis with frequent infections, cystic fibrosis, active TB disease (latent infection excluded), or active non-tuberculous mycobacterial disease (colonization without disease excluded) (A197).

It is not for uncomplicated respiratory disorders, such as routine follow-up of uncomplicated COPD; use the applicable general assessment code instead (A198).

Billing rules

  • Must include the elements of a medical specific re-assessment, or it pays as a lesser assessment (A197).
  • Limited to 6 per patient per physician per 12 months; extra services pay as a lesser assessment (A198).
  • E078 is not payable for the same patient the same day as A570 (A198).
  • Detention (K001) is payable only after 60 minutes (GP30).
  • Can be done by video or phone with an existing patient relationship; add K300A (video) or K301A (phone) (AJ1, A66).

Often billed with

  • Pulmonary function studies or oximetry, payable in addition to the assessment (J3, H4–H6).

Common rejections

  • ADH: OHIP's message is 'Cannot be billed together': E078 was billed the same day as A570, which isn't allowed (A198).
