Code
ADH
Description
Cannot be billed together

Explanation

Updated October 26, 2026

These fee codes can't be billed together. Keep the one the Schedule allows and resubmit.

What it means

ADH means the codes on the claim cannot be billed together. The Schedule treats them as mutually exclusive or overlapping for the same patient and encounter.

Common causes

  • Two codes for overlapping services at one visit
  • A visit code and a service that already includes a visit
  • A code billed with another that the Schedule says it is not payable with

How to fix it

  1. Check the payment rules for both codes in the Schedule of Benefits.
  2. Keep the code that best describes the service and remove the other.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process described under VJ7.

Don't resubmit if

  • You haven't changed the combination of codes.

How APEX handles it
