Complex house call assessment for a frail elderly or housebound patient at home. Payable for the first person seen at a location only.
What it covers
A900 is a primary care service in the patient's home for a patient who is frail elderly or housebound. It must meet at least all the requirements of an intermediate assessment (A6).
Frail elderly means 65 or over with complex medical management needs (which may include polypharmacy), cognitive impairment, age-related reduced mobility or falls, or unexplained functional decline. Housebound means the person has difficulty getting to office-based primary care for medical, physical, cognitive or psychosocial reasons, transportation and other options aren't available or appropriate, and care can be delivered effectively at home (A6).
Billing rules
- Payable only for the first person seen during a single visit to the same location (A6).
- Not payable unless the patient is frail elderly or housebound, and the chart must show both that an intermediate assessment was done and why the patient qualifies (A6).
- Payable in the patient's home or an assisted living or retirement residence; not payable in a long-term care home (A6).
- Not on the Appendix J list, so it can't be billed by video or telephone (A68, AJ1, AJ2).
- FHO: A900A is an in-basket core service (Agreement Schedule 2, p.75) and counts toward the home visit special payment (Agreement p.67, section 8.9).
Often billed with
- Table VI home special visit premiums: travel (B960 weekday daytime, B962 weekday evening) and first person seen (B990, B994), for the first patient seen at the home; the time of the visit must be charted (GP65, GP69, GP75).
- Elective home visits use the B960 and B990 elective column (GP65, GP75).
