Explanation

Plastic surgeon's specific re-assessment of a hospital in-patient: full relevant history and exam of one or more systems.

What it covers

A specific re-assessment requires a full, relevant history and physical exam of one or more systems (GP23).

An admission assessment counts as a specific re-assessment when a surgeon assessed the patient before admission for the same illness for a Z-code (IOP) procedure, or when the same physician assessed and then admitted the patient for the same illness (GP23, GP40). C084 is the in-patient listing for plastic surgery (A181).

Billing rules

  • Limited to 2 per patient per physician per 12 months, except those rendered for hospital admissions; extras are paid as a lesser assessment (GP23).
  • A surgeon can't bill the hospital admission assessment unless it is the major pre-operative visit (SP2, GP40).
  • By video only (not phone), and for in-patients only if you aren't the MRP, no local physician with the expertise is available (documented by the referrer), and the referrer assessed the patient in person within the prior 30 days (Appendix J, A69).
  • For emergency calls and other special visits to in-patients, use the general listings (A-prefix) with special visit premiums instead (A181, GP65–GP78).
