Explanation

Neuro-ophthalmology consultation for a hospital in-patient, under the same conditions as A231 (fellowship training, 4 of 7 exam elements).
What it covers
The in-patient version of A231, subject to the same conditions (A162).
Only for a patient with a neuro-ophthalmological disorder, by an ophthalmologist with neuro-ophthalmology fellowship training, with at least four of these documented: detailed pupillary exam, detailed extraocular motility exam, ocular alignment testing, partial or complete neurological exam, detailed fundus exam, analysis of formal visual fields, and analysis of pertinent diagnostic imaging (A154).
Billing rules
- Same conditions as A231: fellowship training, neuro-ophthalmological disorder, and at least 4 of the 7 exam elements documented (A162, A154).
- Consultation rules and limits apply (GP16–GP18).
- In-patient rules apply (GP40–GP48). For emergency calls and other special visits to in-patients, use the general listings (A-prefix) and special visit premiums instead (A162).
- Age premiums for patients under 16 apply (GP64).
- Not listed in Appendix J, so not payable as a virtual service (AJ1–AJ2).
Common rejections
- ARF: The claim is missing the referring provider's number. A consultation needs a written request that names the referring physician or nurse practitioner and their billing number (GP16).
