Explanation

Concurrent care visit by a consulting cardiologist to a hospital in-patient when the family physician stays the most responsible physician.

What it covers

C608 is a concurrent care visit by a cardiologist to a hospital in-patient, paid per visit (A80).

Concurrent care is any routine assessment in hospital by the consultant after their first major assessment, when the family physician remains the most responsible physician but asks the consultant to continue directive care (GP48).

Billing rules

  • Limited to 4 claims per week in the first week of concurrent care and 2 per week after that; claims over the limit are not payable (GP48).
  • The family physician must remain the MRP and must have asked for your continued directive care (GP48).
  • E083/E084 MRP premiums don't apply, since they are only for visits by the MRP (GP47).
  • Not on the Appendix J virtual care list, so not payable by video or phone (AJ1–AJ3).

Often billed with

  • C101 ICU/CCU premium for each patient seen on a visit to the ICU or CCU, unless a critical care, ventilatory or comprehensive care team fee is claimed (GP103).

Common rejections

  • AHF: OHIP's message is 'Concurrent or Supportive Care Same Period': another concurrent or supportive care claim overlaps the same period for this patient; check the weekly limits and who is the MRP (GP48).
