Code
AHF
Description
Concurrent or supportive care already billed for this period

Explanation

Updated October 26, 2026

AHF means concurrent or supportive care was already claimed for this patient in the same period. Check the weekly limits before resubmitting.

What it means

AHF means OHIP rejected a concurrent or supportive care visit because one has already been claimed for the same patient in the same period.

Concurrent care is a consultant's routine hospital visit after their first major assessment, when the family physician stays the most responsible physician but asks the consultant to keep directing care. Supportive care is a family physician's routine hospital visit to a patient under another physician's care, at the patient's or family's request, for liaison or reassurance. Each is limited to 4 claims in the first week and 2 per week after that. Visits over the limit are not eligible for payment (GP48).

Common causes

  • More concurrent or supportive care visits than the weekly limit allows
  • Concurrent care for this patient already claimed by another physician
  • A wrong service date that places the visit in a week that is already full

How to fix it

  1. Count the concurrent or supportive care visits billed for this patient, by week, starting from the first such visit.
  2. If this visit is over the limit, it isn't payable. Don't resubmit.
  3. If the service date is wrong, correct it.
  4. If you had actually become the patient's most responsible physician, bill the subsequent visit code that matches that role instead.
  5. Resubmit the corrected claim within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • The visit is over the 4-then-2-per-week limit.
  • Another physician already holds concurrent care for the patient and you have no other role in the case.
