Explanation

Updated October 26, 2026

Adds 20% to an eligible procedure fee when an ED physician billing H codes does it on a weekday evening, or a weekend or holiday daytime or evening.

What it covers

E412 increases the procedural fee by 20% for procedures done by an Emergency Department physician on weekday evenings (17:00–24:00) or during daytime and evenings on Saturdays, Sundays and holidays (GP104).

Eligible procedures are non-elective surgical procedures (including fractures and dislocations), obstetrical deliveries, clinical procedures associated with diagnostic radiology, K101, K111, K102, K112, and the listed major invasive procedures (GP103–GP104).

Billing rules

  • Only payable when the ED physician is, at the time, required to submit claims using H-prefix emergency codes (GP104).
  • The procedure must be non-elective, or elective but delayed by an intervening surgical emergency into the premium period (GP104).
  • Nights (00:00–07:00) are E413 (+40%), not E412 (GP104).
  • Physicians other than ED physicians use E409 (+50%) or E410 (+75%) instead (GP104).

Often billed with

  • Non-elective surgical procedures such as laceration repair (Z154, Z177) (GP103–GP104).
  • Listed major invasive procedures such as G211 or G269 (GP104).

Common rejections

  • AD9: A premium cannot be paid alone; E412 must be on the same claim as an eligible procedure (GP104).
