Code
AD9
Description
Premium not allowed alone

Explanation

Updated October 26, 2026

A premium was billed without the service it must go with. Add the base service, or remove the premium, and resubmit.

What it means

AD9 means the premium is not allowed alone. Premiums are paid on top of another service and can't be paid without it.

For example, special visit premiums are only payable with certain consultation, visit or procedure codes; the Ministry's Error Report message 04 says a special visit premium is payable only with a fee code from the general listings.

Common causes

  • A premium billed without its base visit or procedure
  • The base service billed on a separate claim
  • The base service rejected, leaving the premium alone

How to fix it

  1. Check which services the premium can be billed with in the Schedule of Benefits.
  2. Add the base service to the claim, or remove the premium if no qualifying service was provided.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process described under VJ7.

Don't resubmit if

  • No qualifying base service was provided.
