Explanation

Add-on for each additional finger (phalanx) fracture treated at the same time as a phalanx fracture billed with F004 or F005.
What it covers
E558 is listed under phalanx fractures of the hand as 'each additional' (N14). It pays for each additional phalanx fracture when you treat more than one at the same time.
The FHO agreement describes it as each additional phalanx fracture, to F004A (no reduction, rigid immobilization) or F005A (closed reduction) (Agreement p.92).
Billing rules
- Add-on only: bill it with F004 or F005 for the first phalanx fracture (N14; Agreement p.92).
- Open reduction is a separate listing, F007 (N14).
- A cast or corrective splint applied during the first 2 weeks is included in the fracture or dislocation fee; later casts are billed from the cast listings (N7).
- Bill one E558 for each additional phalanx fracture treated (N14).
- Patients under 16: payment is increased by 10% to 30% depending on age (GP64).
- FHO: a Schedule 4 code that counts toward the office procedures special payment when done in your office for enrolled patients (Agreement p.67 §8.7, p.92).
Often billed with
- F004 phalanx fracture, no reduction, rigid immobilization (N14).
- F005 phalanx fracture, closed reduction (N14).
Common rejections
- ADF: The base phalanx fracture code (F004 or F005) is missing, invalid or paid at zero on the claim; E558 is only payable with it (N14).
