Code
ADF
Description
Required related procedure missing, invalid or paid at zero

Explanation

Updated October 26, 2026

The procedure this code depends on is missing, invalid or paid at $0. Fix the related procedure and resubmit.

What it means

ADF means the corresponding procedure is invalid, omitted or paid at zero. The billed code is only payable with another procedure, and OHIP didn't find a valid, paid one.

On the Remittance Advice, the matching explanatory code is DF: the corresponding fee code was not billed or was paid at zero.

Common causes

  • The required procedure wasn't billed
  • The required procedure was rejected or paid $0
  • A different procedure was billed from the one the code must go with

How to fix it

  1. Check the Schedule of Benefits for the procedure this code must be billed with.
  2. Make sure that procedure is on the claim and payable.
  3. If it was rejected, correct its error first, then resubmit both together.
  4. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process described under VJ7.

Don't resubmit if

  • The required procedure wasn't provided.
