Explanation

Updated October 26, 2026

Add-on for unilateral ureterolysis during a major gynaecological operation, except S743.

What it covers

E854 is a unilateral ureterolysis payable with a major gynaecological operative procedure (V1).

It must include surgical definition of the pararectal and paravesical spaces, identification of the uterine artery and vein, and mobilization of the pelvic ureter from the common iliac vessels to the ureterovesical junction (V1).

Billing rules

  • Payable with a major gynaecological operative procedure, except S743 (V1).
  • Not payable for the same patient on the same day as S743, by any surgeon (V9, note 1).
  • All the listed surgical steps must be done; otherwise the ureterolysis isn't E854 (V1).
  • E854 is on the list of codes that take the E676 morbid obesity premium (SP6).

Often billed with

  • S757 abdominal hysterectomy (V8).
  • S763 radical hysterectomy or S727 ovarian debulking (V8, V10).

Common rejections

  • ADF: The base procedure was missing, invalid or paid at zero; this add-on is only payable with a major gynaecological procedure (V1).
