Explanation

Office glucose test, quantitative or semi-quantitative, done in your own office for your own patient, with the result and decision charted.
What it covers
G002 is a quantitative or semi-quantitative glucose test done in the physician's office (J67). It is a Laboratory Medicine in Physician's Office service under Miscellaneous Tests, which any physician may claim (J65, J67).
Codes in the separate Schedule of Benefits for Laboratory Services apply only to private licensed laboratories, not to office testing (J65).
Billing rules
- Insured only when you, or a physician substituting for you, do the test in your own office for your own patient (J65).
- The chart must record the result, your interpretation and the treatment decision based on it, or the test isn't payable (J65).
- Not marked (+), so the G700 sole-visit premium does not apply (J3, J67).
- May be delegated to a trained employee in your office, even when you are not on site, for your own patient; the chart must be dated and name who did it (GP62–GP63).
- For a nursing home or home for the aged patient, G002 is one of the lab tests included in W010 monthly management: when W010 is paid, G002 by any physician that month isn't eligible for payment (GP51–GP52).
- Not on the GP58 list of exceptions, so G002 is not eligible for payment when the same physician renders any counselling service (K013, K033, K040, K041) to the patient that day (GP58).
- FHO: a Schedule 2 core (in-basket) service, and in Schedule 2A for long-term care patients (Agreement p.76, p.83).
Often billed with
- An assessment such as A003 or A007; office lab tests are payable in addition to a consultation or assessment (J3).
- G010 urinalysis without microscopy when also done at the visit, under its own code (J68).
