Office urinalysis without microscopy: one or more parts of a routine urinalysis, done in your own office for your own patient.

What it covers

G010 is one or more parts of a routine urinalysis without microscopy: SG, pH, protein, sugar, haemoglobin, ketones, urobilinogen or bilirubin (J68). When microscopic examination of a centrifuged specimen is included, bill G009.

It is a Laboratory Medicine in Physician's Office service, listed under Miscellaneous Tests (J65, J68).

Billing rules

  • Insured only when you, or a physician substituting for you, do the test in your own office for your own patient (J65).
  • The chart must record the result, your interpretation and the treatment decision based on it, or the test isn't payable (J65).
  • Not marked (+) in its listing, so the G700 sole-visit premium does not apply (J68).
  • May be delegated to a trained employee in your office, even when you are not on site, for your own patient; the chart must name who did it (GP62–GP63).
  • For a nursing home or home for the aged patient under W010 monthly management, office lab tests by any physician that month are included in W010 (GP51).
  • Not insured for monitoring adverse effects of a calorie-restricted weight-loss program (J68).
  • Payable on the same day as a counselling service by the same physician (GP58).
  • FHO: a Schedule 2 core (in-basket) service (Agreement p.76).

Often billed with

  • An assessment such as A001 or A007; office lab tests are payable in addition to a consultation or assessment (J3).
  • Counselling such as K013 the same day (GP58).

Common rejections

  • VJ7: OHIP received the claim more than three months after the service date, so it was rejected as stale-dated (GP9).
  • EH2: The health card version code on the claim doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit.
  • EH4: OHIP's record shows the patient's coverage ended before the service date; confirm the health number and service date, then the patient's coverage.
  • VH8: The date of birth on the claim doesn't match OHIP's record for that health number; correct it and resubmit.
  • R02: OHIP describes this as 'Invalid HSN': the health number on the claim failed OHIP's check; compare it with the patient's card.
