Office wet preparation for fungus, trichomonas or parasites, done in your own office for your own patient, with the result charted.
What it covers
G012 is a wet preparation examined for fungus, trichomonas or parasites in the physician's office (J68). A fungus culture including KOH preparation and smear is a separate code, G011 (J68).
It is a Laboratory Medicine in Physician's Office service under Miscellaneous Tests, which any physician may claim (J65, J68).
Billing rules
- Insured only when you, or a physician substituting for you, do the test in your own office for your own patient (J65).
- The chart must record the result, your interpretation and the treatment decision based on it, or the test isn't payable (J65).
- Not marked (+), so the G700 sole-visit premium does not apply (J3, J68).
- May be delegated to a trained employee in your office, even when you are not on site, for your own patient; the chart must be dated and name who did it (GP62–GP63).
- For a nursing home or home for the aged patient, G012 is one of the lab tests included in W010 monthly management: when W010 is paid, G012 by any physician that month isn't eligible for payment (GP51–GP52).
- Not on the GP58 list of exceptions, so G012 is not eligible for payment when the same physician renders any counselling service (K013, K033, K040, K041) to the patient that day (GP58).
- FHO: a Schedule 2 core (in-basket) service, and in Schedule 2A for long-term care patients (Agreement p.76, p.84).
Often billed with
- An assessment such as A007; office lab tests are payable in addition to a consultation or assessment (J3).
- FHO: Q012A with an after-hours A007A for an enrolled patient; the after-hours premium is paid on the listed visit codes, not on G012 (Agreement p.68 §8.12).
