Explanation

Updated October 26, 2026

Monthly fee for long-term anticoagulant supervision by telephone: monitoring, ordering and reading blood tests, and adjusting the dose.

What it covers

G271 pays for one month of long-term anticoagulant supervision with telephone advice (J8). It includes monitoring the patient on anticoagulant therapy (ordering and interpreting blood tests, asking about complications), adjusting the dose and prescribing other therapy where appropriate, telephone discussion with the patient or representative even when they initiate the call, and arranging related services (J8).

Billing rules

  • Paid per month, for the month's telephone supervision (J8).
  • Included in W010 monthly management of a nursing home or home for the aged patient: when W010 is paid, G271 by any physician that month is part of it (GP51).
  • Payable in addition to an assessment on the same day, as a diagnostic and therapeutic procedure not listed as included in assessments (J3).
  • Not on the GP58 list of services payable the same day as counselling by the same physician (GP58).
  • Not on the Appendix J virtual care lists; telephone advice is already part of the service (J8, AJ1, AJ2).
  • FHO: a Schedule 2 core (in-basket) service, and in Schedule 2A for long-term care enrolled patients (Agreement p.77, p.84).

Often billed with

  • A007 intermediate assessment when the patient is also seen (J3).

Common rejections

  • EH2: The health card version code on the claim doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit.
