Error code
EH2 — Mismatched Version Code
Wrong version code
The health card version code on the claim isn't the one OHIP has on file for this patient; they're often still covered.
What to do: Get the patient's current card or run a health card validation, correct the version code, and resubmit.

Explanation
Updated October 26, 2026
EH2 means the health card version code on the claim doesn't match the Ministry's record. Get the current version code and resubmit.
What it means
Every Ontario health card has a 10-digit health number and, on photo cards, a two-letter version code. The health number stays with the patient for life, but the version code changes when a new card is issued. EH2 means the version code you sent is not the one OHIP has on file for that patient, so the claim was rejected.
The patient is often still covered. The Ministry's health card validation service treats an invalid version code as an invalid card held by an eligible person. The claim usually pays once it carries the right version code.
Common causes
- The patient was issued a replacement card (lost, stolen, damaged, or details changed) and the chart still has the old version code
- The patient renewed their card and the new version code was never recorded
- A typing error in the version code
- The version code was left off a card that has one
How to fix it
- Ask the patient for their current card, or run a health card validation check, and compare the version code with the one on the claim.
- Update the version code in the patient's chart so future claims don't reject the same way.
- If you can't get the card from the patient, physicians registered as health information custodians can request the health number and version code from ServiceOntario's Health Number Release service (the 24x7 Help Desk or form 1265-84).
- If the patient never received a new card, ask them to contact ServiceOntario. Resubmit once a valid version code is available.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You haven't found a new version code. Sending the same one again will reject again.
- A validation check shows the patient had no OHIP coverage on the service date. That is an eligibility problem (for example EH4), not a version code problem.
How APEX handles it
APEX does not resend EH2 claims automatically. It parks them on a separate eligibility (EH2) table with the patient's name, date of birth and the rejected version code, ready for a version-code lookup. If the same claim has another error APEX can fix, the fix is saved with it instead of being sent and rejected again for EH2. See how APEX works · Pricing
