Explanation

Angiograms during cardiac catheterization. At most two: one with the right heart and one with the left heart catheterization.

What it covers

G297 covers angiography during cardiac catheterization, whatever the number of chambers injected (J11).

Cardiac catheterization codes (Z439 to G288) include inserting the catheter (with cutdown and vessel repair if done), placing it, injecting contrast, imaging and interpretation (J9).

Billing rules

  • Only two angiograms may be billed: one per right heart catheterization and one per left heart catheterization (J11).
  • G297 falls in the Z439 to G288 range, so when it is an additional procedure at one sitting it is paid at 50% (J9).
  • A note under the angioplasty listings says: if the anatomy is unknown at the time of the procedure, claim G297 at 50% (J11).
  • It is on the major invasive procedure list for after-hours premiums: add E409 (+50%) or E410 (+75%) when non-elective, or elective but delayed by an emergency into that period; ED physicians billing H codes use E412/E413 instead (GP103–GP104).
  • J021 and J022 (radiology catheter insertion and selective catheterization) are not payable in addition to cardiac catheterization procedures Z439 to G288 (E2).
  • The # prefix means the fee does not include premises, equipment, supplies and personnel at the place where the procedure itself is done (J1, J3).

Often billed with

  • Z439 right heart or Z440/Z441 left heart catheterization (J9, J11).
  • Z442 selective coronary catheterization (J11).
  • E409/E410 after-hours premium when non-elective (GP104).

Common rejections

  • A3H: More angiograms were billed than allowed; only two may be claimed, one per right and one per left heart catheterization (J11).
