Error code
A3H — Maximum number of services reached
Too many services billed
The claim line has more services of this fee code than OHIP accepts for it, so nothing was paid.
What to do: Reduce the number of services to what was provided and allowed, adjust the fee, and resubmit.
- Code
- A3H
- Description
- Maximum number of services reached
Explanation
Updated October 26, 2026
A3H means the claim billed more services of a fee code than OHIP allows. Reduce the number of services to the allowed limit and resubmit.
What it means
The Ministry keeps a Fee Schedule Master (FSM) that sets the maximum number of services it accepts for each fee code. When the number of services on a claim line is more than that maximum, OHIP rejects the claim with A3H. A3I is the same check for X-ray codes.
A3H is an Error Report rejection, so nothing on the claim was paid. When a limit is applied to a paid claim instead, it shows as an explanatory code on the remittance advice, such as M1.
Common causes
- The number of services was set higher than the code allows, for example 02 on a code limited to one.
- Units for a time-based code were counted incorrectly.
- The number of services was keyed wrong.
How to fix it
- Check the Schedule of Benefits for the code's limit and how its units are counted.
- Reduce the number of services to what was provided and allowed, and adjust the fee to match.
- If you believe more services were provided and payable, call the Service Support Contact Centre (1-800-262-6524) before resubmitting. Don't split the services across extra claims to get around the limit.
- Resubmit the corrected claim. If more than three months have passed since the service date and the original claim was sent on time, use the stale-dated claim process described under VJ7.
Don't resubmit if
- The number of services hasn't changed. It will reject again.
How APEX handles it
For family practice claims, APEX resubmits the claim with the number of services set to 1 and the original diagnostic code. See how APEX works · Pricing
