Explanation

Updated October 26, 2026

Professional component of a maximal or submaximal exercise stress ECG. The physician must be in attendance throughout.

What it covers

G319 is the professional component of a maximal stress ECG (to exhaustion, symptoms or ECG changes) or submaximal stress ECG (to target heart rate) on a treadmill or ergometer, with continuous monitoring and resting ECGs before and after (J15). G315 is the technical component.

The professional component includes the clinical assessment immediately before testing (J15), and the interpretation must be done personally by the physician who bills it (J1).

Billing rules

  • A physician must be in attendance at all times during the test (J15).
  • The pre-test clinical assessment is part of G319, not a separate visit (J15).
  • Uninsured for routine annual stress tests in asymptomatic patients whose 10-year coronary heart disease risk is under 10% by a generally accepted method such as the Framingham Risk Score (J15).
  • Not payable for routine preoperative screening for a low-risk procedure or a patient at low risk of perioperative cardiac complications, unless there is another clinical indication (J15).
  • Oximetry J323 and J332 are not payable with G319 (H6).

Often billed with

  • G315 technical component for the same test (J15).

Common rejections

  • ARF: OHIP describes this as 'Missing Physician Referring Number': the claim went in without a referring provider number where OHIP's claim edits expected one. If another provider requested the test, add their billing number and resubmit.
