Nephrologist's all-inclusive daily (per diem) care of a kidney transplant recipient from the 2nd to the 10th day after the transplant.

What it covers

The nephrological component of renal transplantation is being in constant or periodic attendance after the transplant to provide all aspects of care to the renal transplant patient: an initial consultation or assessment and any later assessments, ongoing monitoring and intervening as needed (J69).

G408 is the per diem for the 2nd to 10th day inclusive; day 1 is G412 and days 11–21 are G409 (J69).

Billing rules

• Per diem, days 2 to 10 inclusive after transplantation (J69).
• Includes complete patient care, so consultations and visits for the patient during this period are not billed separately (J69).
• Not payable after transplantation of an organ other than the kidney (J69).
• Marked #: the fee doesn't include premises, equipment, supplies or staff, which are funded by the hospital or ICHSC (J1, GP13).
