Nephrologist's all-inclusive daily (per diem) care of a kidney transplant recipient from the 11th to the 21st day after the transplant.

What it covers

The nephrological component of renal transplantation is being in constant or periodic attendance after the transplant to provide all aspects of care to the renal transplant patient: an initial consultation or assessment and any later assessments, ongoing monitoring and intervening as needed (J69).

G409 is the per diem for the 11th to 21st day inclusive; earlier days are G412 (day 1) and G408 (days 2–10) (J69).

Billing rules

• Per diem, days 11 to 21 inclusive after transplantation (J69).
• Includes complete patient care, so consultations and visits for the patient during this period are not billed separately (J69).
• Not payable after transplantation of an organ other than the kidney (J69).
• Marked #: the fee doesn't include premises, equipment, supplies or staff, which are funded by the hospital or ICHSC (J1, GP13).
