• D&T procedures are otherwise payable in addition to a consultation or assessment unless listed as included (J3).
• Add G700 when a (+) procedure is the sole reason for the visit, once per visit; not in hospital, and not when an assessment is also paid and the physician has a financial interest in the facility (J3).
• Tonography (G856/G433) includes tonometry (J90).
